Factbox: U.S. healthcare by the numbers

(Reuters) - The U.S. Supreme Court's healthcare ruling on Thursday has far-reaching implications for a healthcare system that, according to the Paris-based Organization for Economic Cooperation and Development, is highest in spending but just 31st in providing coverage to its people among the OECD's 34 members.

The following information ranking the United States by category against the OECD's other members comes from the November publication "Health at a Glance 2011 - OECD Indicators."

* 1st in Spending - Annual healthcare spending totals $2.6 trillion, equal to 17.9 percent of U.S. annual gross domestic product, or $8,402 for every man, woman and child.

* 1st in Good Health Self-Assessments - 90 percent of U.S. adults aged 15 and older describe themselves as being in good health versus an OECD average of 69.1 percent.

* 1st in Obesity - More than one-third of American adults are obese, up from 15 percent in 1980.

* 2nd in Prevalence of Diabetes - 10.3 percent of the U.S. population suffers from diabetes, surpassed only by Mexico's 10.8 percent. The OECD average is 6.5 percent.

* 3rd out of nine in Waiting Time for Specialists - Out of nine countries from Europe, North America and Australia and New Zealand, the United States has the third shortest waiting time for specialist appointments at 20 weeks; Germany and Switzerland had shorter waiting periods.

* 4th in Preventing Death from Stroke - The United States ranks behind Israel, Switzerland and France with 32 stroke-related deaths per 100,000 people.

* 7th in Cancer Incidence - Cancer afflicts more than 300 people per 100,000 in the United States, compared with an OECD average of 261 per 100,000.

* 9th in Preventing Death from Cancer - At 185 deaths per 100,000, the United States is well above an OECD average of 208 per 100,000.

* 10th in Number of Practicing Nurses - 10.8 per 1,000 population versus an OECD average of 8.4 per 1,000.

* 11th of 11 in Unmet Need for Care Due to Cost - The United States ranks last among 11 OECD countries in its ability to provide affordable care: 39 percent of people with below-average income and 20 percent of people with above-average income report foregoing a doctor visit or prescription because of the cost.

* 25th in Preventing Death from Heart Disease - At 129 deaths per 100,000 people, the U.S. heart disease mortality rate is below an OECD average of 117 per 100,000.

* 27th in Life Expectancy - Americans can expect to live 78.2 years on average, below the OECD average and just behind Slovenia and Chile.

* 29th in Number of Practicing Doctors - The United States has 2.4 practicing doctors per 1,000 population, placing it below an OECD average of 3.1.

* 29th in Doctor Consultations - At 3.9 annual doctor visits per capita, the United States leads only Ireland, Mexico, Sweden and Chile versus an OECD average of 6.5 percent per capita.

* 30th out of 39 in Hospital Beds - 3.1 per 1,000 population.

* 30th in Medical Graduates - 6.5 per 100,000 population, ahead of only France, Japan and Israel. The OECD average is 9.9 per 100,000 population.

* 31st in Health Coverage - An estimated 81 percent of Americans are covered by private or government health insurance, placing the country ahead of only Turkey, Mexico and Chile; 25 OECD countries cover 99 percent or more of their citizens.

* 31st out of 40 in Infant Mortality - 6.5 babies die per thousand live births in the United States, placing the country behind Poland and the Slovak Republic and below an OECD average performance of 4.4 per thousand live births.

* 31st in Preventing Premature Death - The number of years lost in the United States to premature death is surpassed only by Hungary, Mexico and Russia. The main causes are accidents, violence, cancer and circulatory disease.

Its members are Australia, Austria, Belgium, Canada, Chile, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Ireland, Israel, Italy, Japan, Luxembourg, Mexico, Netherlands, New Zealand, Norway, Poland, Portugal, Slovak Republic, Slovenia, South Korea, Spain, Sweden, Switzerland, Turkey, the United Kingdom and the United States.

How do you buy a melon?

Dear Gerry,

Trust your nose. If it smells quite sweet, it will be sweet. Check for ripeness by pressing the dime-sized round at the melon's end. Hard means it needs several days on the kitchen counter. If it has a bit of give, the melon should be ready. Never buy melons with soft spots, withered skins, musty aromas or no aroma at all. And if possible, buy organic ones for better flavor.

The food police side of my nature has to add that tricky bacteria and dirt can cling to the melon's skin. When you cut into it, your knife carries them onto the flesh of the fruit. So scrub melons with a vegetable brush and hot water running water before cutting into them. Always dry it well to avoid deterioration.

Russell Brand slams TV host for divorce talk

Russell Brand may be comfortable talking about his divorce from Katy Perry, that is, unless his mother is in the room.

On an episode of UK's "The Graham Norton Show" that aired Friday, the?"Rock of Ages" actor, who brought along his mother, Barbara, to the taping, lost his patience when host Graham Norton brought up his failed marriage.

"My mum is here. She got upset when you dragged up my marriage. I saw her cry, Graham," the comedic actor fumed. "That's the reality because it's real people, Graham, that's my real mum. I have come here to promote a film and you made my mum cry. I don't see you as that sort of person, Graham."

"I don't see myself as that kind of person ..." Norton, 49, said in defense while insisting that he only inquired due to the fact that Perry was on his show last week.

But Brand wasn't ready to finish schooling Norton on his wrongdoing. "What I would have done, mate, in your position is come up to me before the show and gone, 'Eh, listen it's a bit odd your ex-missus was on last week. I might mention it, is that OK?'"

Meanwhile, just days before his appearance on?"The Graham Norton" show, Brand appeared to be in much different spirits over his personal affairs.

"You just have to have acceptance of things," he told "Lorraine's"?Lorraine Kelly during a Thursday interview about his 17-month failed marriage. "She is a person that I love, but now this is a new time."

Romney says immigration decision complicates issue

(AP) ? Softening his rhetoric on immigration, likely Republican presidential nominee Mitt Romney said Friday that the status of younger illegal immigrants was important and should be addressed with legislation. He would not say if he would reverse President Barack Obama's decision to stop deporting some who came to the U.S. as children.

The former Massachusetts governor's statement echoed the tempered reaction of Sen. Marco Rubio, R-Fla., a Cuban-American who is working on a bill that would allow some illegal immigrants a way to get some form of legal status. Rubio, seen as a top contender for running mate on a Romney ticket, also referred to the decision as a short-term answer and criticized the policy as ignoring the Constitution and going around Congress.

Romney's reaction to Obama's announcement was significantly softer in tone than his rhetoric during the contentious GOP primary campaign. Ahead of January's Iowa caucuses, when he faced the challenge of winning over the right-wing base of the GOP, he vowed to veto the so-called DREAM Act, a bill backed by Democrats that would have created a path to citizenship for illegal immigrants brought to the U.S. as children. Instead of emphasizing the plight of illegal immigrants, Romney focused on the consequences illegal immigration has for U.S. jobs.

On Friday, Romney spoke to reporters outside his campaign bus on the opening day of a six-state tour and a few hours after Obama's announcement. He would not answer shouted questions about whether he would reverse the order if elected president.

"It's an important matter to be considered and should be solved on a long-term bias so they know what their future would be in this country," Romney said. Obama's executive order to allow some illegal immigrants to obtain work permits and stay in the U.S. legally was problematic, he said, because "an executive order, of course, is a short-term matter. It can be reversed by subsequent presidents."

Romney has previously supported a path to legal status for illegal immigrants who serve in the military.

The changed tone reflects the demands of the general election. Republicans are working to woo Hispanic voters who have supported Democrats in previous presidential elections. They're particularly critical, advisers say, in states like Nevada and Colorado, where significant Mexican and Central American populations are especially worried about immigration policy.

Romney's campaign is working on a significant strategy to woo Hispanics. So far, Romney's team has simply translated its English-language ads into Spanish, but the campaign is also working on a separate plan aimed specifically at addressing Hispanic concerns.

At the "welcome to our school!" ceremony, Hades was like "Bitches, please! you ain't gonna open this school! I ain't gonna letcha so close it down before i do!" And then Zeus was like "KK, so everyone get to the common room so that you don't panic and whatevs.

So everyone went to the common room and Doug said "let's all play a game!"

Sator decided to flirt with Victoria who was like "Bitch! You ain't gon touch this! None you boyz evah gonna touch this!" and then Siren decided to flirt with her.

So Doug was like "Let's all say who we izz, who our 'rentz be, and somethang ineresting about ourselvezz" and all the peeps began to play.

There's also gonna be a dance that will end real badly (YAY!!) and everyone is encouraged to find a date.

So Siren was like "Yo yo, Victoria! You wanna be my date to da dance?" and victoria was like, "Umm, cha!"

And Sator was like, "Yo seth! Wuzzap??" And then Sapphira was all like "Yo, dat be ma roomie, is you botherin' him? Hey, anyone wanna be my date for the dance????" and Seth was like, "...no." And she was like T-T and walked away.

Choosing a sports supplement can be very confusing for most people, especially when you consider just how many different products are available on the market these days. Are you looking at packing on the pounds or do you simply need something that will allow you to push out that extra few reps? Making this choice should give you a more focused range of products to look through, in the end making your decision easier. Finding the right sports supplement for you is all about figuring out what you need and then finding a product that is able to supply it.

Minnesota Senate to get hit with another Brodkorb defense legal bill ...

The Minnesota Senate still hasn?t paid the $46,000 bill for its defense lawyers' initial work in the Michael Brodkorb wrongful termination case, and another big bill is on the way.

DFL Minority Leader Tom Bakk, who, along with Republican majority leader Dave Senjem, must sign the check, is responsible for the delayed payment on the first bill.

?I will not sign off on this bill until there is a meeting of the rules committee,? he said again Wednesday.? The rules committee has jurisdiction over the Senate?s internal management.

Furthermore, he said, ?I have to believe there?s an interest clock running on the late payment.?

There will be. According to the terms of the agreement with the Larkin Hoffman law firm and attorney Dayle Nolan, the bill increases by 1.5 percent a month?? about 700 dollars ?beyond the June 21 due date.

Secretary of the Senate Cal Ludeman agrees. ?It?s probably going to take a rules committee meeting, but if it gets done by the 21st of June, we don?t need to worry about the interest fees.?

The Senate, though, will need to worry about the next legal bill.

The fees in the current bill covered only the period ending in the middle of March.? They do not cover the hours Nolan sat in a Senate ethics hearing involving Sen. Geoff Michel?s handling of the Brodkorb dismissal. It later was revealed that Brodkorb was the unnamed staffer who was involved in a personal relationship with his supervisor, then Republican Majority Leader Amy Koch.?

The Michel hearings in late March and April ended without a decision. They incurred a minimum of 10 hours of legal time, followed by subsequent legal work. Nolan?s hourly rate is $330.

Furthermore, the Senate is only authorized to cover legal fees of up to $50,000.? The rules committee, an 11-member panel chaired by Senjem, would need to authorize another appropriation to cover the second bill, which the Senate expects to receive shortly.

All of those factors lend force to Bakk?s demand for transparency in what taxpayers will end up paying.

? In January, Brodkorb and his attorneys began tryuing to negotiate a settlement of his dismissal. He has yet to actually file a legal claim, waiting to get a go-head from the Equal Employment Opportunity Commission.

But, Bakk has no doubt that Brodkorb will, and he foresees the legal wrangling dragging on for years.?

So Bakk is demanding a vote from the seven Republican and four DFL members. ?I want the rules committee to approve the bill,? he said.? ?There needs to be a process.?

That means a bigger need for more housing and plenty of other services at military bases. The increased population of bases has been a cause for concern. The National Research Council?s Transportation Research Board was asked by Congress to observe the effects the base consolidations would have on traffic and transport on bases.

The study warned that traffic in general was going to be a big problem for bases where jobs and military personnel increased due to other base closures. Sparing additional personnel, money and resources to constantly cope with traffic blockages, accidents and dangers isn?t feasible for many bases already stretched to their limit. (Boston) A recent study conducted by researchers at Boston Medical Center (BMC) shows that patients receiving "take home" methadone maintenance treatment (MMT) were less likely to be admitted to the hospital as compared to those not receiving take home doses. The findings, which are published online in the Journal of Addiction Medicine, demonstrate the potential benefits of successful addiction treatment, including better overall health and decreased health care utilization.

This research was led by Alexander Walley, MD, MSc, physician in general internal medicine at BMC and medical director of the Opioid Treatment Program at the Boston Public Health Commission. Daniel Alford, MD, MPH, medical director of the Office-based Opioid Treatment (OBOT) at BMC and associate professor of medicine at Boston University School of Medicine (BUSM), and Jeffrey Samet, MD, MA, MPH, chief of general internal medicine at BMC and professor of medicine and community health sciences at BUSM and Boston University School of Public Health, respectively, were the study's senior co-authors.

Among people addicted to opioids, methadone maintenance treatment prevents symptoms of withdrawal, blocks the effects of illicit opioids, like heroin, and reduces cravings. Methadone dosing is individualized and research has found that doses greater than 80 milligrams are more effective than lower doses in curbing cravings.

Federal and state regulations require methadone maintenance patients to attend the clinic daily to receive medication at the beginning of treatment. Patients that exhibit treatment successes, including regular attendance at clinic and counseling sessions, as well passing urine screenings for illicit drug use, are rewarded with "take home" methadone doses. These patients receive medication that they may take at home instead of coming into the clinic. Previous studies have shown that receiving "take home" doses and receiving doses of 80 milligrams or more are associated with improved addiction treatment outcomes, but the impact of these factors on hospitalizations was unknown.

To explore these questions, the researchers performed a retrospective analysis of 138 patients enrolled in the Boston Public Health Commission's MMT program for a period of two years between 2006 and 2008. The results showed that patients receiving "take home" methadone doses were substantially less likely to be admitted to the hospital with 74 percent lower odds of hospitalization. The data also showed no evidence that the dose of methadone was associated with hospitalization.

"These study results add to the mounting evidence that patients who are successful in their addiction treatment are also improving their overall health, which could result in a reduction of health care utilization," said Walley, who also is an assistant professor of medicine at BUSM. "We have demonstrated an association between take home status and hospitalization, which is an important medical and health system cost outcome that needs to be considered as we further explore the benefits of addiction treatment to the patient and the health care system as a whole."

(Boston) A recent study conducted by researchers at Boston Medical Center (BMC) shows that patients receiving "take home" methadone maintenance treatment (MMT) were less likely to be admitted to the hospital as compared to those not receiving take home doses. The findings, which are published online in the Journal of Addiction Medicine, demonstrate the potential benefits of successful addiction treatment, including better overall health and decreased health care utilization.

This research was led by Alexander Walley, MD, MSc, physician in general internal medicine at BMC and medical director of the Opioid Treatment Program at the Boston Public Health Commission. Daniel Alford, MD, MPH, medical director of the Office-based Opioid Treatment (OBOT) at BMC and associate professor of medicine at Boston University School of Medicine (BUSM), and Jeffrey Samet, MD, MA, MPH, chief of general internal medicine at BMC and professor of medicine and community health sciences at BUSM and Boston University School of Public Health, respectively, were the study's senior co-authors.

Among people addicted to opioids, methadone maintenance treatment prevents symptoms of withdrawal, blocks the effects of illicit opioids, like heroin, and reduces cravings. Methadone dosing is individualized and research has found that doses greater than 80 milligrams are more effective than lower doses in curbing cravings.

Federal and state regulations require methadone maintenance patients to attend the clinic daily to receive medication at the beginning of treatment. Patients that exhibit treatment successes, including regular attendance at clinic and counseling sessions, as well passing urine screenings for illicit drug use, are rewarded with "take home" methadone doses. These patients receive medication that they may take at home instead of coming into the clinic. Previous studies have shown that receiving "take home" doses and receiving doses of 80 milligrams or more are associated with improved addiction treatment outcomes, but the impact of these factors on hospitalizations was unknown.

To explore these questions, the researchers performed a retrospective analysis of 138 patients enrolled in the Boston Public Health Commission's MMT program for a period of two years between 2006 and 2008. The results showed that patients receiving "take home" methadone doses were substantially less likely to be admitted to the hospital with 74 percent lower odds of hospitalization. The data also showed no evidence that the dose of methadone was associated with hospitalization.

"These study results add to the mounting evidence that patients who are successful in their addiction treatment are also improving their overall health, which could result in a reduction of health care utilization," said Walley, who also is an assistant professor of medicine at BUSM. "We have demonstrated an association between take home status and hospitalization, which is an important medical and health system cost outcome that needs to be considered as we further explore the benefits of addiction treatment to the patient and the health care system as a whole."

Apple MacBook Pro 15-inch (Retina Display)


The Apple MacBook Pro 15-inch (Retina Display) is the laptop you want if you care about performance, thinness, and the screen. It's not the vaunted "15-inch MacBook Air" that was rumored prior to 2012's WWDC?it's better, thanks to an up-to-date components, super-thin chassis, and impressive battery life. This "next-generation" MacBook Pro hasn't just caught up to the thin and powerful Windows laptops and ultrabooks on the market; it has surpassed them to become the high-end choice for media professionals, enthusiasts, and general Mac fans alike. As such, the MacBook Pro is our new Editors' Choice for high-end desktop replacement laptop PCs.

Design and Features
The MacBook Pro 15-inch (Retina Display) carries the same Jony Ive?design DNA as previous systems?it's all aluminum unibody chassis, glass, and black plastic for the keyboard. In fact, it still looks like a MacBook Pro, which is good news, because the cachet of the Apple ID is part of the reason people go nuts for the company's products. It even feels similar to previous models in your hand, though it's noticeably thinner and lighter (4.46 pounds versus 5.6 pounds); if you've held a 13-inch MacBook Air, you're not too far off.

The 15.4-inch screen now looks more seamless, if that's possible. The bezel around it is black, but unlike on the MacBook Air it's of a piece with the screen glass. The screen electronics are built into the glass, which helps the laptop's thin profile. Apple mentioned that the screen is less prone to glare than was the case with previous MacBooks, but the glare is still visible when you're viewing a black background and if you're really picky. If you want a matte-finish screen, for now you'll have to go with the updated MacBook Pro with Ivy Bridge.

The Retina display itself is glorious. The resolution is 2,880 by 1,800, which sounds like a lot, but text is scaled so it doesn't look too small. Instead of making the letters smaller like on the iPhone 4 or 4S (to see this effect, use one to visit a non-mobile-optomized Website), Apple kept the font sizes consistent with what you'd expect in the real world and just made them smoother. In contrast, text on a MacBook Air looks smooth from your seat, but the individual letters are still jaggy close up. Text on the new MacBook Pro looks smooth from both far away and close up, as if it were laser printed on paper.

The real magic is when you view photos (and high-res video). You can view images straight from your camera and they will look more like printed images than electronic ones. Look at a geometric form, like a picket fence in front of a yellow wall, and the lines look smooth, not jaggy. Likewise, a 1,920-by-1,080 HD video takes up a relatively small portion of the screen at full resolution, leaving the video editor with lots of space for timelines, toolbars, and other interface items. It's almost like having a dual 20-inch-screen setup in a 15-inch diagonal space. When playing back 1080p video full screen, the improved IPS display exhibits rich colors, deep blacks, and a generally pleasant viewing experience. It really is like having a large-screen HDTV you can rest on your lap.

If there's any drawback to the Retina display, it's that all of your existing Mac applications will have to be updated for it (kind of like what happened with the iPhone 4/4S and latest iPad). Apple-sourced apps like Safari, Final Cut Pro, and Aperture look terrific, but non-optimized apps like Google Chrome will show upscaled and jaggy fonts. It's a problem that's likely to go away as more developers update their programs, but it's an annoyance right now.

The power button has moved to the upper right of the keyboard proper, like it is on the MacBook Air. The island-style keyboard has the same feel as the MacBook Air; key travel feels shallower than the previous MacBook Pro. The function keys match those of the MacBook Air, which may be a hang-up for people with older-generation MacBook Pros, particularly ones made before the advent of Mission Control and the Launchpad in Mac OS X Lion. The backlighting is everything we expect from a MacBook, clearly visible in a darkened room. After several straight hours of playing back video during our battery rundown test the bottom of the system was still cool to the touch, demonstrating the new chassis' cooling capabilties.

There is a full-size HDMI port on the side of the laptop. The new MagSafe 2 port is wider yet shorter than the previous style, so you'll need an adapter for existing LED Cinema and Apple Thunderbolt Displays, as well as older power adapters. (Newly purchased Thunderbolt Displays will come with the MagSafe 2 adapter.) The USB 3.0 ports aren't colored blue like they are on some Windows PCs, but because there aren't any USB 2.0 ports on this MacBook Pro, you won't need color coding to tell the difference.

Copying a 1.22GB test folder from a USB 3.0 drive took 21 seconds, which is half the time we needed to copy the same folder using a USB 2.0 drive on the previous MacBook Pro. Speedier drives using the Thunderbolt interface are likely to be even faster. Two Thunderbolt ports are a boon for the video editor: You can connect up to 14 devices, seven devices per port. The system can also support at least two Thunderbolt Displays for more screen real estate. An SDXC slot and a headphone audio jack with headset support round out the ports.

One thing you won't find on the new MacBook Pro is a DVD SuperDrive: The optical drive has been eliminated to benefit the new profile, which is 0.71 inch (18mm) at the thickest point (meeting the spec for 13.3-inch or smaller ultrabooks, and slimmer than the 21mm requirement for 14-inch or larger ultrabooks). The MacBook Pro is the same height as both (11- and 13-inch) MacBook Air models at their thickest points, an impressive feat. The slim profile prevents Apple from using Ethernet and FireWire 800 ports (they simply won't fit); if you need either, you'll need to buy adapter cables (at $29 each). Using a Thunderbolt Display is also an option, as it has built-in FireWire 800 and Ethernet. On the plus side, the MacBook Pro includes 802.11a/b/g/n Wi-Fi with 2.4GHz and 5GHz support, as well as Bluetooth 4.0, so you'll be able to connect to almost any hotspot or wireless audio device.

The system comes with Mac OS X Lion 10.7.4, so you'll get all the same iLife apps and familiar Mac OS interface. Systems purchased now will be eligible for a free upgrade to Mac OS X Mountain Lion (Mac OS X 10.8 in all but name) when it is released in a month or so. Mountain Lion will introduce iMessage, Notification Center, Power Nap, AirPlay, and lots of other iOS-like features to the MacBook Pro and other Macs.
